What is Texas HCS?

Home and Community-based Services (HCS) is a Texas Medicaid waiver program administered by the Texas Health and Human Services Commission (HHSC). It provides eligible people with intellectual or developmental disabilities a range of supports in community settings, such as their own or a family member’s home, or a small group home.

HCS is designed to support eligible Texans with intellectual disability or a related condition in community settings. HCS supports supplement other available services and natural supports, including family and community resources. HHSC determines eligibility, and services must be authorized in the individual’s plan.

People interested in HCS can contact the local intellectual and developmental disability authority (LIDDA) serving their county to ask about the interest list and enrollment process. The timing of an offer depends on individual need, placement date, and available program funding. The LIDDA can guide applicants and families through next steps.

06 / COORDINATION

A Connected

Collaboration with individuals, families, service coordinators, and care professionals helps keep support focused on the person’s goals.

Support shaped around the individual. We listen to what matters to each person—including daily routines, food and activity preferences, communication style, relationships, community interests, and personal goals. When possible and authorized, the support plan can be tailored to those preferences and reviewed with the individual and their team.

Additional HCS supports to discuss

Depending on the person’s plan and provider availability, additional supports may be considered to reflect individual preferences and goals.

Respite

Planned short-term support that gives unpaid caregivers a break while maintaining the individual’s routines and preferences.

Behavioral supports

Individualized strategies that support communication, emotional well-being, safety, and participation in daily life.

Skills for daily living

Support to practice personal routines, household activities, self-advocacy, and greater independence at the person’s pace.

Adaptive aids & home access

When approved, adaptive equipment or minor home modifications may help a person do more safely and comfortably.

Employment & community goals

Depending on the service plan, HCS may include supports connected to employment readiness, supported employment, and community participation.

Therapy & wellness coordination

HCS plans may include nursing or therapy supports when authorized, with coordination across the person’s care team.

Additional waiver services are not automatic or available to everyone. They must meet HHSC program rules, be authorized in the individual’s plan, and be within the provider’s approved service capacity. We will discuss the person’s preferences and confirm which supports can be provided or coordinated.
